There are no instructions, but the pepakura model file (PDO) is included (see the description). Read the pepakura section of this to see how to use it:

Might I ask, how do you put a download file different from the preview image? I tried, but I didn't figure out how to do it... *blush*

its not something you're able to do so much as something you have to do, whenever you upload a file it can't display (i.e. anything other than an image) you have to upload a preview picture with it
